The Problem

The people you serve reach you through phone calls, paper forms and drop-ins. Appointments get missed, contact details go stale, and follow-up for outcome reporting means staff tracking people down months later.

The Build

A web or mobile app built around what the people you serve actually need to do: check in, confirm appointments, update their contact details, complete intake or follow-up surveys, and hear from staff.

What they enter flows straight into your case management system, so follow-up data arrives on its own and outcome reports draw on answers people gave directly. Volunteers and partner agencies can get their own views too, for referrals, schedules and updates.

The Approach

It starts with the people who will use it: conversations with the people you serve and frontline staff about how they stay in touch today, what phones and data plans they rely on, and where people fall out of contact.

The app is built for an older phone and limited data, written in plain language, and handles privacy from the start, because it holds information about people who often have good reason to guard it.
